1.Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2017) which can be
downloaded at www.csc.gov.ph together with Work Experience Sheet attachment;
2.Performance rating in the last rating period (if applicable);
3.Photocopy of certificate of eligibility/rating/license;
4.Photocopy of Transcipt of Records and other pertinent documents to include Service Record/Certificate of Employment and Certificate of
Relevant Trainings and Seminars attended.
5.Other expectations: For INST1-4-2021, preferably Licensed Professional Teacher and with NSTP Teaching Experience
For INST1-23-2021, preferably with experience in teaching General Education Curriculum; has experience in handling faculty training &
formation and a Licensed Professional or with Second Level Eligibility
